What are dental implants?

Dental implants are small rod-like posts that act as a base for replacement teeth. They are placed into the jawbone and act like roots to support a tooth, denture or bridge.

The implant screw fits into the bone and carries a clip (abutment) onto which the dental crown fits. Titanium is the usual material, because it has no ill effect on the human body and bonds readily with new bone that grows into the titanium surface. With good oral hygiene and care, implants should last a lifetime.

Treatment options

Immediate placement & immediate loading

Where conditions allow, the implant can be placed the same day the tooth is removed.

Immediate placement

In this process the implant is placed at the same time as the tooth is extracted. It is often advantageous to place the implant immediately. The titanium fixture is then allowed to heal under the gum for about three months or longer, depending on the case — healing takes longer if grafting or tissue reconstruction is necessary. Once healed, the crown can be fitted.

In some instances it is possible to fit a replacement tooth to the titanium insert straight away. This is known as “immediate loading”.

Immediate loading

Immediate loading is possible when sufficient stability is achieved during surgery. When attaching an immediate tooth we have to consider osseointegration — the bonding of the titanium insert to the jawbone — which takes around 2 to 3 months. Biting pressure should ideally be low during this period.

The definitive porcelain crown is attached to the implant once it has bonded with the bone.

The single tooth implant process

1 · Extraction and socket cleaning

The tooth is first removed and the socket cleaned, ensuring the residual socket is free from infection or debris so the fixture can be placed.

2 · Assessing bone integrity

We assess the bone at the site to decide whether the implant can be placed at that time — with or without additional tissue augmentation — or whether the fixture should be placed after the socket heals.

3 · Bone or gum graft if needed

A bone graft or gum graft may be necessary if the outer aspect of the bone socket is fragile after extraction, or if the socket is larger than the titanium insert. The residual gap is filled with bovine bone minerals, which also thickens the bone. A gum graft is used for thin, transparent or dark gums to thicken the tissues and improve resistance. These are usually addressed during a single visit to our Upper Clapton practice.

4 · Crown fitting

Once the implant has integrated, the porcelain crown is fitted — shaped and shaded to match your natural teeth.

Earlier extraction

If you need to replace a single tooth that was extracted earlier, the same procedure is followed. The dentist will assess the risk factors and decide whether grafting is required. An immediate crown can be fitted at the same time as the new implant.
